Conference Expense Claims

Thank you for attending the Annual Ocular Melanoma UK Conference.

Please download and complete the Conference Expense Form using the link below.

Once you have filled in your details, save a copy of the form to your computer before sending it back.

This page allows you to claim back agreed travel expenses related to your attendance. You can download the fillable PDF form below, complete it electronically, and return it by email or post using the details provided.

To make sure your edits are saved correctly, please download the file to your computer first and open it in Adobe Acrobat Reader or another PDF editor. If you fill it in through your web browser, your information may not save properly. When finished, go to File, Save As and keep a copy for your records.

Once completed, please email your form and receipts to accounts@omuk.org or post to Ocular Melanoma UK, PO Box 6136, Slough, Berkshire, SL3 7LU.

Terms & Conditions:

  • Reimbursement will be against complete details and scanned receipts only.
  • Please note that we do NOT cover bank charges due to faulty bank details.
  • We have budgetted upto £100 for economy-class travel only.
  • If your travel costs more than £100, please indicate above whether you would like us to reimburse you the full amount and we will reimburse once all claims are received and if sponsorship funds allow us to do so. 
  • Expense claims must be received by 30th November 2025
